Sign, date, and bring the policy form to your first appointment with us.
Making an online payment is fast, easy, and secure.
We offer an in-house dental plan to encourage good oral health and reduce the rising cost of insurance. This program is available to all our patients, whether individuals or entire families. Our goal is to provide quality dental care that is affordable for everyone.
We are out of network with most insurance plans, and do not accept Medicaid.
Our dental office sends computerized billing to your insurer. This means you won’t need to bring any insurance forms with you. However, we will need you to bring your insurance card with you on your first visit so that we may have it on record. To simplify paperwork, we create a standard insurance form your carrier should accept. This helps us coordinate your insurance benefits with more speed and accuracy.
Your co-pay will be due at the time of service. To avoid any surprises, learn what your policy covers before your appointment.
If you don’t have dental insurance, your balance will be due at the time of service. We accept cash, personal checks, Visa, Mastercard, and Discover. You may also apply for an extended payment plan through CareCredit. Learn more by calling our dental office or visiting CareCredit.com.
